The site says about the PFD's
" Paragraph 7 of Schedule 5, Coroners and Justice Act 2009, provides coroners with the duty to make reports to a person, organisation, local authority or government department or agency where the coroner believes that action should be taken to prevent future deaths."

There are many more Coroner issued Preventable Death Notices issued to Camden NHS bodies than listed here. 2
psychiatric hospitals and the HQ of the local Mental Health Trust are located in Camden. The rate of suicide is so high that the local coroner Mary Hassell has written to the NHS Trust flagging the same repeated concerns being ignored. Despite this not a single full Safeguarding Review has EVER been held by LB Camden into preventable deaths of people known to or under the care of Camden and Islington NHS Foundation Trust who are a partner agency and where 30% of community staff are actually LB Camden employees located there
